Add Individuals to Student Record Release Information

This article walks through how to add a person to the student records release information to give them access to either all or some parts of a student’s student record at PCCC.

Instructions

Step 1. Go to “Student Records Release” Page in Self-Service

Log into your PCCC student portal and click “Self-Service” in the side navigation menu on the left of the page.

If you cannot see the labels of the sidebar, you can expand it by clicking the icon with the right-facing arrow.

In Self-Service, click on your name at the top right of the page and then select Student Records Release in the dropdown menu.

 

This will take you to the “Student Records Release Information” page, where you can add and view the individuals who have access to your student records.

Please read the information at the top of the screen, which talks about privacy and access to your student records.

 

Step 2. Add a Person (family, friend, or third party) to your Student Records Release Information.

To add a person that you’ll allow access to view your student records, click the Add Person/Relationship button. Afterwards, please fill the blank spaces with the following required information using the identity of the person you are giving student access to:

  • First Name

  • Last Name

  • Relationship: who the person is to you.

You can also add the following optional information:

  • 4-digit pin

Be sure to tell that person what the pin is. They’ll need it in order to access your records.

  • Start date: The first day that the person can request access to your student records.

  • End date: The last day that the person can request access to your student records.

Access

After you fill in that information, please select the amount of access that you are giving to the person you are adding.

  • Allow Complete Access: This choice will give the person access to all parts of your student record. You will not be able to choose individual parts of your record.

  • Allow Select Access: This option will allow you to choose which information that person can access out of all your student records. You can select these individual parts of your record by check marking each one.

Disclosure Agreement

Finally, please checkmark the disclosure agreement to agree with it. The agreement states you are giving PCCC permission to disclose your student records to the person you are adding to your student records release information.

After that, click Submit.
