My 1098-T form is incorrect. How do I correct it?

Solution

• Name of the school you attended

• Your (student's) name as reported to the school

• A telephone number where you can be reached

• Last 4 digits of your social security number

• Amount disputed and attached report showing amounts billed or paid for the reporting period
